Effectiveness of a simulation-based training program for accidental out-of-hospital deliveries

Abstract
Background: This study aimed to determine the effectiveness of a simulation-based accidental out-of-hospital delivery education program for paramedics.

Methods: This study was designed as a single group of pre- and post-education experiments for paramedics and as a simulation-based education program for two days. The participants were currently working as 119 paramedics, and 36 out of 41 people expressed their intention to voluntarily participate in the study. For two days, the students participated in an educational program based on a simulation that was jointly developed by a gynecologist, a paramedicine professor, and a current emergency medicine doctor. In order to verify the effectiveness of accidental out-of-hospital delivery simulation education, we analyzed changes in knowledge, skills, and clinical skill self-confidence after the educational intervention.

Results: After the education-based simulation program, the paramedics¡¯ knowledge and clinical skills scores increased significantly (p<0.003, p<0.001, respectively). In the simulation training, participants' confidence in their clinical skills increased to 3.84¡¾0.81 (possible range:1-5) from before training (p<0.001). The lower area of the highest score was 'understanding of patient status due to delivery' (4.85¡¾1.92) and the lower area was 'checking and interpreting the fetal heart rate at the correct location' (3.21¡¾0.84) (p<0.05)

Conclusion: Simulation-based accidental out-of-hospital delivery education programs for paramedics were found to help increase self-confidence in clinical skills and improve theoretical knowledge and essential skills in emergency delivery.
